Is it the hook in old christmas bulb ?

I’d say it looks like either an old cloth covered button or and old christmas decoration

The top of Christmas tree decoration … you’d put the hook thru the wire and hang it on the branch.
I haven’t been by in a while to guess… but I’m pretty sure this is an old Christmas ornament!
